Hometown Waiver Solutions is dedicated to providing quality services to individuals eligible for Medicaid Waiver Services in Indiana
Hometown Waiver Solutions has over 30 years of combined experience providing services to individuals with Developmental Disabilities. We serve individuals who are on the Indiana Medicaid Waiver Program. More Specifically:
CIH WAIVER to include:
- Residential Habilitation (RH10, RH20)
- Day Habilitation, Individual (DHI)
- Respite (RSPO)
- Transportation (Level 1&2)
- Remote Supports (RS1)
- Structured Family Caregiving (Level 1,2,3)
FSW Waiver to include:
- Participant Assistance & Care (PAC)
- Day Habilitation, Individual (DHI)
- Respite (RSPO)
- Transportation (Level 1&2)
- Remote Supports (RS1)
H and W Waiver to include:
- Structured Family Caregiving (Level 1,2,3)
Our services will range from serving as much as 24 hours a day to several hours per week. We provide services statewide.
